EXTENDED HEALTH CARE

Eligibility

Extended Health Care is an extension of your medical coverage and is designed to protect the Member and his/her dependents against many of the expenses incurred during a period of illness.

There is no annual deductible. In-Canada expenses are reimbursed at 90% unless otherwise indicated and all In-Canada eligible expenses will be reimbursed up to a lifetime maximum of $1,000,000 if under age 80 and to a lifetime maximum of $20,000 if age 80 or older.

Out-of-Province Benefits

Out of Province/Canada Emergency Medical Travel Insurance cover age is provided to eligible Members and their dependents under age 80 up to a maximum of $5,000,000 per coverage period. Those 80 years and older do not have Out of Province/Canada Emergency Medical Travel Insurance coverage but may use what is remaining of their $20,000 lifetime maximum towards such expenses. It is recommended that if you or a dependent is 80 years or older that you purchase Travel Insurance.

How a Claim is Made

Use your pay-direct card for prescription drug claims, participating paramedical practitioner claims and eye examinations.  If you weren’t able to use your pay-direct card, you can submit your claim for reimbursement directly through the D.A. Townley My Claims portal or mobile app.

Alternatively, you can use the Extended Health Benefits Claim. Please include your Group Plan number (70682) and your certificate/Client ID, along with copies of your receipts.

If you are coordinating the claim payment with your spouse’s health plan, you should include the primary carrier’s payment statement.

Your fully completed claim form and receipts can be emailed to health@datownley.com or faxed to (604) 299-8136 or mailed/dropped off to the Plan Administrator
